To get your application assessed by the ANZCCP HREC please register with your details below. The price to submit a new AP application is $990 (including GST) for a new application or $590 (including GST) for a renewal.

The fee includes ANZCCP membership for 2 years plus access for 1 year to the Authorised Prescriber course. This fee must be paid on submitting registration below. Your application will not be assessed prior to completion of all requirements.

Once the Medicinal Cannabis Authorised Prescriber Application is formally approved, the AP is approved for a period of 2 years.

Please note that generally applications from medical practitioners with **non-practising, limited, student, provisional registration (requiring supervised practice), or conditions placed on their registration will not be considered for the Authorised Prescriber scheme.

Please have the following documents ready to complete your application form

New AP appplication

• 4 x TGA SAS B Approval Letters (At least 2 Schedule 4 (Category 1) and 2 Schedule 8 (Category 2-5), two of which should be for Dried Herb (if you wish to include it in your application)
• Patient Informed Consent Form – Using the ANZCCP Consent form template

• Evidence of additional professional education – if applicable

AP Renewal

• Previous HREC AP approval letter
• Previous HREC AP amendment letter(s) if they were issued
• All TGA 6 monthly reports for previous HREC approval period
